Please complete this Water Activities Authorization Form to allow your child to participate in water activities (e.g., water play, splash days, or swimming) at Children's Discovery & Learning Center LLC. This form ensures we have your consent, safety information, and emergency contact details, in compliance with Louisiana Department of Education (LDOE) requirements for nonpublic schools.
